NEW PRACTICE MEMBERS
If you’re NEW to Rise, book a NEW PRACTICE MEMBER APPOINTMENT. Choose the appropriate initial visit type and complete your paperwork 12 hours in advance. If you don’t complete your paperwork, you may be asked to reschedule.
EXISTING PRACTICE MEMBERS
- For an adjustment, schedule a FOLLOW UP
- For rehab, nutrition, lifestyle, or mindset coaching book in addition to your adjustment book an EXTENDED FOLLOW UP
- To book a Follow Up for more than one person use GROUP BOOKING
- We offer 4 types of REASSESSMENT Appointments. Choose the appropriate visit type and complete your paperwork 12 hours in advance.
NUTRITION
You can book with our nutrition team here.
POLICIES
- CANCELLATION POLICY: 12 hours notice is required to cancel or move your appointment. Late cancellations and no-shows will incur a $40 fee.
- MEDICAL INSURANCE: We DO NOT work directly with any medical Insurance company, but can provide a Superbill you can use for reimbursement. Credit Card + FSA/HSA are accepted forms of payment
- MEDICARE & AUTO ACCIDENTS: We DO NOT accept NEW Practice Members who have Medicare or who are seeking care for injuries sustained in an auto accident.
- If you book an appointment online and the incorrect appointment type is booked, we will move your visit to another provider at the same time. If we are unable to accommodate, we’ll reach out to you to reschedule
Please Note: Dr. Nicole is currently on Maternity leave. Look for her schedule to re-open in early 2025.

New Practice Members
If you are NEW to our office, please schedule a New Practice Member Appointment to establish care at Rise. If you’re under 18 years of age, a parent or guardian is required to be present for your New Practice Member Appointment. After booking your visit, please complete the initial paperwork attached to your appointment. Please note, at this time we are NOT accepting New Practice Members whose primary insurance is Medicare or New Practice Members who are seeking care for injuries sustained in an auto accident. Please contact the office if you’re in need of a referral.
Existing Practice Members
Here you'll find a list of appointments offered for established Practice Members. For a standard treatment and a continuation of care, please schedule a Follow Up Appointment. If you’d like to spend more time doing rehab, nutrition, or movement-based work during your visit, please schedule an Extended Follow Up Visit. If you have a new problem or would like your progress to be evaluated, please schedule a reassessment visit.
Family & Group Visits
